Anterior Bridge

Aproximatly 40 year old male presents with failing dentition and difficulty eating.

#7 and #10 need to be extracted.

Patient does not wish to have SCRP. Patient elects to have implant retained lower denture.

Sinus Bump

PT Present with #J broken crown. This x-ray was taken 1 year ago.

Pre-op photo

post extraction, buccal wall is intact.

Partial sinus bump. Only 1 mm additional height obtained. Additional height was obtained using osteotome and mallet.

Post bone graft and membrane placement. Primary closure was not obtained.

Post op X-ray. Sinus was raised 2-3mm for a total of 9mm of bone. Sinus will again be raised during implant placement in 16 weeks.

Lava bridge

Pt present with unesthetic smile. #9 is fractured and needs to be extracted.

Perio defect above #9 is noted. Patient does not have a high smile line but traps food under temp.

Defect in in height as well as ridge width.

Bridge comes back and esthetics are much better. Patient is very happy.

Patient elects to have bone graft for #9

Incision is places palatal to crest of ridge, flap elevated.

Collagen membrane placed into flap. Perioglass bone graft placed.

2x 3.0 chromic gut sutures.

Bridge is cemented with temp bond.

Occlution is checked.

Kicked in mouth!

Patient was kicked in the mouth during a fight. Teeth are not mobile. #24 was fractured into pulp. Root canal, post and core build up.

Collapsed vertical.

Patient is not happy with look and fit of dentures

Patient was placed on TENS unit for 1 hour before final impression and bite on wax rims

5 Unit PFM Bridge

 

35 y.o. male patient who wished to have #9, and #10 replaced. Both teeth were lost to trauma years ago. Due to time constraints, the patient did not wish to have implants. A PFM bridge from #7 through #11 was fabricated.

Note the rotation for #8 and the position of #7.

Abutments were prepared to allow PFM to be in the proper position.

Temporary bridge made with Jet and a suckdown matrix from the mock wax- up.

3 unit PFM

Before #10 was ext.

75 y.o. male presents for replacement of #10. The tooth required endo treatment and could not be restored.

PFM #9 was cut off. Decay on mesial was removed, and Ketac silver was used to build up abutment. #11 had a large composite restoration and decay under margin. Carious exposure discovered a necrotic pulp. After endo therapy, Para post and ketac silver was used as a build up.

Patient with temporary bridge. The color was too light, however it was the darkest shade available in the clinic. Temps made with JET.

Custom stain, Shade was selected under natural light. This picture was taken with camera's flash.

After 1 week in temporary cement, pt wanted 8, 9 shortened to better match adjacent teeth.

Marginal gingiva of #11 has receded because of impingement of temporary crown. Gingiva looks healthy and has grown to fill defect.

PFM Crowns: # 7, 8, 10

 

Patient presents with #4, #6-12 only maxillary dentition.

Tx Plan: #7,8,10 endo, Para post and core build up. PFM's, Maxillary partial denture

# 4 was extracted due to the mobility and a severe dilacerated root tip. At this point all endo is complete.

Para post and composite core build up. Temps made and occlusion distributed among remaining teeth.

Abutments prepared for PFM with lingual in metal.

Shade chosen

Temps made with Jet acrylic. Porcelain was in lab for 1 month. Temp #10 came off 2 days before appointment.

Bisque bake, Vita shade B3. It looked very light during try in and needed custom staining.

Lingual in metal due to deep overbite. Metal as thin as .4mm in some areas.

Custom staining with an orange modifier. Brown was added to distal of #7 to create a shadow and blend with #6.

I was not able to capture the shade of the teeth due to the brightness of the ring light. However, the color was a perfect match. So much so that several classmates could not guess which teeth were PFM!

Invisalign

 

Invisalign Case: Pt wished to close diastama between #8-9 aprox 4mm.

Composite attachments bonded to #5, 12, 20, 21, 28, 29.

Almost Done! 3 trays left= 9 weeks

Patient is very happy with progress.
